Healthcare facilities ask more of their building components than almost any other project type. Walls, ceilings, and the access points within them must satisfy safety regulations, infection control requirements, acoustic performance targets, and patient experience goals — often in the same corridor, sometimes in the same room.
Access doors are a small piece of that puzzle, but they're one where a wrong specification creates real consequences: a failed inspection, an infection control breach, or a patient safety incident. When specifying access doors for healthcare, there are several key considerations that deserve careful attention.
Anti-Ligature: More Than a Product Category
Ligature resistance is one of the first design requirements that comes up in behavioral health environments — psychiatric units, crisis stabilization spaces, emergency departments serving mental health patients, and similar settings. The FGI Guidelines define a ligature-resistant environment as one without points where a cord, rope, or fabric can be looped to create an attachment point that may result in self-harm. That definition applies to everything in patient-accessible spaces, including access doors.
Standard access panels fail this test almost by default. Exposed hinges, protruding handles, and sharp frame edges are all potential attachment points. Anti-ligature access door design addresses each of these: concealed hinges, radiused corners, gasketed perimeters, and tamper-resistant hardware.

Airtight Performance: Tested Numbers, Not Just a Gasket
Operating rooms, isolation rooms, sterile processing areas, and BSL-3 laboratory spaces all depend on controlled air environments. An access door that compromises the envelope integrity of those spaces undermines the entire HVAC and filtration system. In an operating room or infectious disease unit, that's a clinical failure, not just a performance shortcoming.
The distinction that matters here is tested performance versus a spec that simply says "gasketed." A lot of doors have gaskets. Very few have documented air infiltration test data.
What to specify: The Acudor ADWT is an airtight/watertight flush access door that has been tested per ASTM E 283 (air infiltration: less than 0.01 cfm/sq ft) and ASTM E 331 (water penetration: no leakage at 15.05 psf). Acoustical Performance: A Real Problem with Real Products to Solve It
Patient privacy is a HIPAA requirement, and the acoustic performance of a partition is only as good as its weakest component. In practice, that weak component is often the access door — an uninsulated panel in an otherwise rated wall assembly can drop STC performance significantly at that location.
The fix isn't complicated, but it requires specifying the right product rather than defaulting to a standard panel.
What to specify: Acudor's acoustical access door line — the ACF-2064, ACD-2064, and ACF-2064-DW — are all tested and certified to STC 64 / OITC 54 (ACF models) and STC 64 / OITC 53 (ACD model). The ACF-2064 is a flush steel door with a 2" acoustical mineral liner and mineral-filled vinyl barrier; the ACD-2064 is a recessed version with a 5/8" drywall face for a finish that blends into the surrounding wall. For locations where the partition also needs to be fire-rated, the FW-5050-ACF is a 1.5-hour fire-rated insulated door with an STC 29 / OITC 24 acoustic rating — covering both requirements in a single product.
Matching the door to the partition rating is the key step. A corridor wall rated to STC 50 warrants a different door spec than an exam room divider at STC 45. Pull the partition schedule and work from there.
Aesthetics: Why It's a Clinical Consideration, Not Just a Design One
The FGI behavioral health design guidelines explicitly call for environments that appear comfortable and residential, not institutional. The concern is grounded in research on how
physical environments affect patient behavior, anxiety levels, and cooperation with care. An environment that looks like a correctional facility tends to produce outcomes consistent with one.
Access doors contribute to this dynamic in a subtle but real way. A visible metal panel with exposed screws and a utilitarian frame registers as institutional, especially in a patient room or behavioral health space. The fix is usually specification, not cost.
What to specify: For patient-accessible areas, flush and recessed panels — such as Acudor's DW-5058, GFRG line, or drywall-bead panels that accept finish coats — allow access doors to disappear into the surrounding surface. In clinical support spaces, staff corridors, and mechanical areas, standard painted steel panels are entirely appropriate and cost-effective. The aesthetic specification decision should match the application and environment, not apply one product across the board.
One Specification Decision at a Time
Healthcare access door schedules tend to get treated as commodity line items. The projects where that approach creates problems are usually the ones where critical details were overlooked for the sake of convenience – a psychiatric unit got a standard panel, a clean room got a gasketed door without tested performance data, or an acoustically sensitive partition has an uninsulated panel at every access point.
The right product does exist for each of these applications. The work is in asking the question before the schedule is finalized, not after the inspection.
